You will be invited by your practice for an annual review with your GP or Practice Nurse with Diabetes expertise. This review will cover blood test results, medication and will address your experience of living with diabetes. It is important for you to share any anxieties with your health care professional.
It is likely that your Practice Nurse and GP will want to see you more frequently than the formal annual review described above; it is not uncommon for people to need regular access to their practice team to monitor Diabetes control. You will agree with your healthcare team a strategy for Diabetes management and a method for keeping in touch, this could be via phone, email or text message if a face to face appointment is not convenient (subject to your practice’s procedure).
Once a year you will be invited for Diabetes Eye Screening. This may be at your practice or a local health centre. This test is essential for ensuring your eyes are healthy. If you are unable to attend this examination please ensure you discuss this with your Practice Nurse or GP or call the Diabetes Eye Screening service for further advice.
