We are engaging with clinicians, therapists, care providers, patients and others in designing a new model of care. This will be done using the experience of patients and by looking outside our area for examples from elsewhere.
A Design Group has been set up with a small group of clinicians, social care workers and patient representatives who will meet three times during March, April and May. In between their meetings, the views of wider groups of patients, staff and the public will be gathered through workshops and surveys so that the Design Group can benefit from the views or others. This is the just one area of care that we want to improve by taking a completely new approach. Transforming care will challenge us all.
This is not an easy undertaking and we need your help to get it right.
We want to start a big conversation including people who work in local services, patients, carers and family, voluntary sector organisations and members of the public. There are several ways you can get involved:

We will be seeking views through short surveys on Health Connect. You can register by clicking here. The surveys will be short and will be open for a few weeks at a time so we can use the feedback at each stage.
