Bracknell Forest Council and Bracknell and Ascot Clinical Commissioning Group are working jointly to support and advise people and their carers, with the aim of helping them stop long-term conditions deteriorating to the point there is a need for emergency hospital admissions and to help improve quality of life. A key part of this is the Helping You Stay Independent Guide, which is a directory of local support available to help people stay healthy and happy.
The Guide, now in its forth edition has been refreshed for 2015/16. Project Managers from partner and stakeholder organisations have each updated the existing entries.
The Guide has new entries including:
- NHS Choosewell (including the Urgent Care Centre)
- Falls Prevention Advisory Service
- The Rapid Assessment Community Clinic (RACC)
- Young Carers
- Safe Place Scheme
- Social inclusion
- Patient Advice and Complaints (PALS and PACT)
